Abstract
Community Health Workers (CHWs) connect individuals and communities with healthcare systems, resources, and social services. This project examined how CHWs impact patient outcomes, health equity, and healthcare systems through a literature review and qualitative interviews with CHWs at Henry Ford Health. Findings showed that CHWs improve patient outcomes, increase health equity, and provide positive financial impacts. The research also identified the need for greater CHW recognition, reimbursement, and integration within healthcare systems.
